This role is integral to supporting frail and elderly participants within the comfort of their homes. The ideal candidate will conduct thorough assessments, deliver skilled nursing interventions, and collaborate with interdisciplinary team ( IDT) members to create individualized care plans.

Job specification

• Perform comprehensive home health nursing assessments to evaluate participant needs, including medical equipment and personal preferences.
• Actively participate in Interdisciplinary Team ( IDT) meetings to contribute to care plan development.
• Deliver and accurately document nursing interventions as outlined in participant care plans.
• Maintain a safe, healthy environment and respond promptly to physician orders and urgent medical needs.
• Administer medications, perform tests and treatments as prescribed, and monitor patient response.
• Educate participants, caregivers, and families on disease processes, self- care techniques, and preventive measures.

Candidate requirements

• Graduate of an Accredited School of Nursing .
• Unencumbered RN License in the state of California.
• Minimum of one ( 1) year of nursing experience in a clinical setting, preferably with elderly or frail populations.
• Home health agency experience is preferred.
• Must have a reliable means of transportation to visit patients in their homes.
