[job_card.job_description]Education : Master of Social Work (MSW) from an accredited institution. Licensure : Must possess an active and unrestricted New York State LCSW license. Experience : New graduates are welcome and encouraged to apply. Technical Skills : Proficiency with telehealth platforms and electronic health records (EHR) systems is a plus. Soft Skills : Strong empathy, excellent communication and interpersonal skills, cultural competence, and the ability to work independently. Conduct psychosocial assessments and provide evidence-based therapeutic interventions to a diverse patient population. Develop, implement, and monitor individualized treatment plans. Maintain timely and accurate clinical documentation in accordance with practice policies. Collaborate effectively with a multi-disciplinary healthcare team to ensure comprehensive patient care. Adhere to all ethical and legal standards of clinical social work practice.
