Employment Type : Full-Time

Salary Range : $80,000 - $110,000 annually

Position Overview :

We are seeking a dedicated and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our team. This full-time position will involve providing high-quality care to patients at both Encompass Health Rehabilitation Hospital and Shah Medical Clinic in Sioux Falls. The ideal candidate will have a strong background in rehabilitation and internal medicine, with a commitment to patient-centered care.

Key Responsibilities :

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Collaborate with multidisciplinary teams, including physicians, therapists, and nursing staff, to coordinate patient care.
• Provide ongoing medical management and adjust treatment plans as necessary.
• Educate patients and their families about health conditions, treatment plans, and preventive care.
• Participate in quality improvement initiatives and contribute to the development of best practices.

Encompass Health Rehabilitation Hospital of Sioux Falls is a 40-bed, freestanding inpatient rehabilitation hospital dedicated to helping patients regain independence after life-altering illnesses or injuries.
