This role is integral to our mission of providing top-tier medical care across various group homes and facilities. This position is ideal for professionals passionate about making a significant impact in healthcare settings outside of the traditional hospital framework. The candidate for this role will be required to perform rounds and administer care directly at our partner facilities. Please note that this is not a remote job and requires on-site presence at the assigned group homes and facilities.
As a Nurse Practitioner at Clarius Medical Group, PLLC, you will play a crucial role in delivering compassionate and high-quality medical care to our clients in group homes or facility-based settings. You will collaborate with a team of healthcare professionals to manage and maintain the health and well-being of our clients, ensuring a continuum of care that corresponds with our clinic's standards.
Duties and Responsibilities
- Conduct regular on-site visits to partner facilities and group homes to assess patient health and implement care plans.
- Participate in the creation and revision of individualized care plans in collaboration with other healthcare professionals.
- Manage chronic health conditions and respond to acute health changes among patients.
- Perform medication reconciliation and management during each visit to ensure accurate treatment continuity.
- Coordinate with nurses and staff at facilities to provide timely and effective care.
- Maintain detailed and comprehensive documentation of patient care activities and outcomes in eClinicalWorks.
- Work collaboratively with the billing and care team at the clinic to ensure all services are accounted for and properly documented.
- Communicate proactively with patients' families or guardians, keeping them informed of health statuses and care decisions.
- Participate in scheduled team meetings to discuss patient care, facility issues, and compliance with health and safety regulations.
- Stay updated on the latest healthcare practices, techniques, and policies relevant to group home and facility care.
Requirements
Current licensure as an Advanced Practice Registered Nurse (APRN) or Nurse Practitioner (NP) in Texas.Proven experience in long-term care, skilled nursing facilities, or home-based care settings is preferred.Must have reliable transportation and the ability to travel to multiple locations as required.Demonstrated ability to work autonomously and manage a significant caseload effectively.Strong organizational and communication skills, with an emphasis on timely and empathetic patient and family engagement.Experience in using eClinicalWorks healthcare software is preferred but not mandatory.Commitment to continuous professional development and adherence to ethical medical practices.
