Home Health Intake Coordinator

A company is looking for an Intake Coordinator for Home Health, working from home.

Key Responsibilities

Serve as the initial point of contact for potential home health patients and manage new referrals

Act as a liaison between patients, families, and referral sources while ensuring follow-ups

Collaborate closely with Home Health nurses to facilitate a seamless intake process

Required Qualifications, Training, and Education

Minimum of 5 years of telephonic experience in a healthcare setting

3-5 years of experience using the Epic healthcare system

High School Diploma or GED required

2 years of recent relevant experience preferred

Knowledge of medical terminology and coding systems (CPT, HCPCS, ICD-9)
