[job_card.job_description]Conduct intake assessments and complete documentation within 48 hours Gather and integrate information from multiple sources (e.g., self-reports, family, medical history) Use motivational interviewing to assess readiness for change Provide accurate information about available services and supports Initiate person-centered planning and develop safety plans as needed Refer clients for further evaluation or services when necessary Promote group and support services Maintain timely and accurate documentation Participate in clinical supervision, consultations, and team meetings Uphold professional ethics and agency policies Education : Master’s degree in Social Work, Psychology, or Counseling – Required Licensure : Must hold one of the following active Michigan licenses : LLMSW, LLPC, TLLP, LMSW, LPC, LLP Experience : Minimum 1 year post-Master’s experience with adults with severe mental illness – Required Must obtain QMHP and QIDDP credentials within 120 days of hire Must possess or be actively pursuing CAADC or MCBAP Development Plan within 6 weeks of hire Medical, Dental, and Vision Insurance 401(k) Retirement Plan Paid Time Off (PTO) Paid Holidays : New Year’s Eve & Day, MLK Day, Good Friday (1⁄2 day), Memorial Day, Juneteenth, Independence Day, Labor Day, Thanksgiving & Black Friday, Christmas Eve & Day Flexible scheduling and a supportive, team-oriented environment

Social Worker Intake Therapist (Licensed or Limited License)
Location : Southgate, MI
Schedule : Monday–Friday, 8 : 30 a.m. to 5 : 00 p.m. (Some evenings / weekends required)
Compensation :
- Full License : $28.90 – $38.89 / hour
- Limited License : $26.68 – $35.91 / hour
Mileage Reimbursement : $0.64 / mile (IRS standard)
Training : Paid by employer
