Experience : Minimum 1 year of Certified Home Care experience and 1 year of Case Management experience

Must-Have :

• 1 year of Certified Home Care experience
• 1 year of Case Management experience
• Ability to travel to assigned cases
• Weekend rotations once every 6 weeks

Description :

As a Home Health RN, you will be responsible for assessing patient conditions.

You will implement care plans and provide ongoing education and support to patients and their families.

Patient care is delivered within organizational philosophy, policy, and standards of community health practice.

You will perform admission processes and case management responsibilities in accordance with established policies and standards of practice.

Thorough and timely electronic documentation and interdisciplinary communications are required.

You will work in various communities and neighborhoods, delivering high-quality care to patients in their homes.
