California Licensed Practical Nurse

A company is looking for a Licensed Practical Nurse (LPN) with California licensure for a remote position on a day shift.

Key Responsibilities

Manage patient census with a focus on closing gaps in patient care

Identify and coordinate community resources to enhance patient care

Provide patient education on managing chronic conditions and perform medication management

Required Qualifications

Active California nursing license (LPN)

At least 3 years of direct patient-facing work experience

Technical aptitude with Microsoft Office Suite

Ability to work in a remote, HIPAA-compliant environment

Experience with Epic and bilingual skills are a plus
