Responsibilities

We are looking for CNAs to join our skilled nursing teams at Community Subacute & Transitional Care Center! As a CNA on our team youll provide direct and indirect care to our diverse patient population under the direction of an RN / LVN. As an empathetic caregiver you will assist patients with daily living activities including but not limited to : personal care wound care feeding bathing and communicating with family.

When a patient needs ongoing care subacute care steps in. Community Subacute & Transitional Care Center is a 106-bed facility providing inpatient care for chronic subacute conditions such as coma or other complex illness that require 24-hour medical supervision.

Patients whose care requires ventilator respiratory wound IV hospice rehabilitation services and / or general nursing can receive treatment thats consistent with an acute care centers effectiveness while in a comfortable long-term care setting.
