Conducts initial visits pending hospice referrals in acute care facility or home to identify needs of the patient.Be aware of each individual disease management before meeting with patients. Electronically documents visits / care in timely manner, following policy and procedure.Determines plan of care for skilled and unskilled care needs based on assessment findings. Revises plan of care as goals are achieved or client's status changes. Reassesses the client's needs based on regulatory requirements and client's status.Utilizes care protocol appropriately at all times. Determines appropriate frequency and treatments based on physician's orders and assessment findings. Follows through on identified problems until resolution is achieved, promoting positive outcomes.Provides documentations for scheduler regarding the level of heath care provider needed to meet the client's needs and regulatory standards.Serves as liaison to other health professionals participating in the client's care. Observes client's status and reports changes to the physician and the clinical supervisor.Implements skilled nursing interventions with the participation of the client and / or family based on physician's orders and plan of care.Teaches, supervises and counsels client and family regarding home nursing procedures and other care needs appropriate to the client's condition.Initiates diagnostic, preventative and rehabilitative nursing procedures as appropriate to the client's care and safety.Evaluates the care the client receives from hospice personnel through supervisory visits and provides input to the supervisor.Evaluate eligibility for hospice services. Continues, refers or terminates care as indicated and documents actions as required. Coordinates care with patient / caregiver.Completes discharge documentation and submits within time frames set by policy.Attendance at staff meetings and in-services and performs other duties as assigned.The employer for this position is stated in the job posting.
