Social Care Network Nutritional Counselor

A community-based wellness program is seeking a Nutritional Counselor to support a holistic, strengths-based approach to health, recovery, and wellness. Programs include residential and outpatient addiction treatment, recovery services, prevention education, and community programming. Services are culturally competent, evidence-based, and designed to enhance resiliency, strengthen communities, and support lasting wellness.
The Nutritional Counselor provides individualized dietary guidance and education to NYS Medicaid participants to promote optimal health and manage medical conditions through nutrition. This role involves assessing nutritional needs, developing personalized meal plans, and supporting clients in achieving their health goals.
Key Responsibilities
- Conduct outreach to Medicaid participants referred for nutritional services.
- Perform comprehensive nutritional assessments based on medical history, lifestyle, and dietary habits.
- Develop and implement personalized nutrition plans tailored to individual goals.
- Educate clients on healthy eating habits, food labels, portion control, and nutrient-dense choices.
- Track participant progress and adjust plans as needed.
- Collaborate with healthcare providers, social workers, and wellness teams to ensure integrated care.
- Maintain accurate and confidential participant records in the case management system.
- Provide group workshops on nutrition-related topics.
- Stay current on nutrition science, dietary guidelines, and public health trends.
- Build relationships with community partners and contribute to coordinated support.
- Attend required meetings, trainings, and program events.
- Adhere to all program, SCN, and NYS policies and procedures.
- Perform other duties as needed.
Qualifications
Bachelor's degree in Nutrition, Dietetics, Public Health, or related field (Master's preferred).Registered Dietitian (RD) or Certified Nutrition Specialist (CNS) required.At least 2 years of experience in nutritional counseling or clinical nutrition.Medicaid-approved provider preferred.Knowledge of chronic disease management (e.g., diabetes, hypertension, obesity).Ability to motivate and support clients through behavior change.Knowledge of social care systems, community resources, and health-related social needs.Familiarity with Medicaid eligibility, benefits, and referral processes.Strong communication, interpersonal, and active listening skills.Technological proficiency with data entry, recordkeeping, and digital tools.Ability to work independently and collaboratively within an interdisciplinary team.Additional Information
Some local travel required for community site visits and events(monthly MetroCard or equivalent provided).Must maintain professionalism in sensitive and confidential situations.Flexibility required as enhanced care services expand.Role will begin in one community wellness center and will eventually extend to additional centers in the Lower East Side.
