2. Skilled Nursing / long term care experience - since October 2019 (PDPM experience)

3. Rehab Optima (Net Health)

4. Negative COVID 19 test required (usually within 7-10 days of start)

5. COVID-19 Vaccination card / documentation-no exceptions
