Assess home care patients to identify their physical, psychosocial, and environmental needs. Complete required assessments and paperwork accurately and in accordance with agency policies. Communicate significant findings and changes to the Clinical Manager and physician, documenting all communications and interventions Supervise and provide direction to home health aides and LPNs / LVNs to maintain the quality and continuity of services. Participate in on-call rotation and emergency call as per agency policy. Basic Cardiac Life Support certification must be obtained within 30 days of employment. Valid Driver's License. Active RN license. Associate Degree in Nursing.

Job Summary and Qualifications
As a Registered Nurse Care Manager, you will provide coordinated skilled nursing care to patients of all ages in their homes. You will collaborate with an interdisciplinary team to establish patient goals, ensuring high-quality care under the supervision of the Clinical Manager.
