Physical Therapist (Sign On Bonus : $20,000)

The Physical Therapist is responsible for evaluating / assessing needs of patients and formulating treatment plans. S / he provides therapy services defined in treatment plans and works cooperatively with physicians. The Physical Therapist may supervise physical therapy assistant(s).

ESSENTIAL JOB FUNCTIONS :

• Consults with patients to learn about their physical condition and symptoms.
• Provides manual therapy as part of treatment plan when indicated.
• Assesses and evaluates therapeutic / rehabilitative / functional status, and participation in the development of the total plan of care.
• Instructs patients and families / caregivers in the use and care of therapeutic appliances.
• Determines priority needs for physical therapy.
• Reports to physician patient's reaction to treatment or changes in condition and document in patient record (EHR).
• Initiates physical therapy program and coordinates with family members on home programs, exercise, and case conference.
• Identifies patient and family / caregiver needs for other home health services and refers as necessary.
• Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.
• Performs other relevant job duties as required by manager / supervisor.
• Complies with federal and state regulations.
• Complies with APTA code of ethics.
• Complies with continuation education requirements.
