Per Diem Licensed Practical Nurse (LPN) – Job Description
Position Summary :
We are seeking compassionate and dependable Per Diem Licensed Practical Nurses (LPNs) to join our staffing pool. Per diem nurses provide flexible coverage for shifts at skilled nursing facilities, rehabilitation centers, and other healthcare settings. The LPN will deliver safe, high-quality patient care under the supervision of a Registered Nurse (RN) or physician, while ensuring compliance with facility policies and state regulations.
Key Responsibilities :
Provide direct nursing care to residents, including administering medications, treatments, and wound care.
Monitor patient conditions, document observations, and report changes to supervising RN or physician.
Assist with admissions, discharges, and patient transfers.
Maintain accurate and timely medical records, charts, and reports.
Support activities of daily living (ADLs), including mobility, hygiene, and nutrition needs.
Collaborate with CNAs, RNs, physicians, and facility staff to ensure continuity of care.
Educate residents and families on care plans, treatments, and health management.
Uphold infection control, safety, and facility compliance standards.
Qualifications :
Current and valid Licensed Practical Nurse (LPN) license in the state of practice.
At least 6 months of nursing experience in a long-term care, rehabilitation, or acute care setting (preferred).
Current CPR / BLS certification.
Strong clinical, assessment, and communication skills.
Ability to adapt quickly in different facility environments.
Dependable, punctual, and able to work flexible per diem shifts (days, evenings, nights, weekends, holidays).
Work Schedule & Compensation :
Per Diem / As-Needed – Flexible scheduling based on facility needs and nurse availability.
Competitive hourly rates with shift differential for nights, weekends, and holidays.
