Key Responsibilities:

• Participate in interdisciplinary team discussions to assess, plan, coordinate, and evaluate resident care.
• Document resident conditions and nursing needs accurately and promptly, while reporting significant observations and reactions.
• Oversee nursing assistant assignments, adapting based on resident census and care requirements.
• Administer treatments and monitor medication reactions, ensuring proper documentation.
• Manage the preparation, administration, and charting of all medications.
• Initiate and execute physician orders effectively.
• Maintain clear communication with families regarding changes in medications and resident status.
• Establish and uphold protocols that ensure timely updates to care plans, reflecting each resident's unique needs.
• Facilitate admissions, discharges, and transfers to ensure high-quality customer service.

Qualifications:

• Graduate of an accredited nursing program for Practical Nursing.
• Current Practical Nurse license and any additional state-required certifications.
