Job Summary :

In this role, you will implement and monitor patient care plans, ensuring well-coordinated care through collaboration with various healthcare professionals. You will assess and address discharge planning needs while applying evidence-based practices tailored to patients' specific clinical and age-related needs.

Key Responsibilities :

• Implement and monitor effective patient care plans, documenting and communicating patient conditions.
• Coordinate care across multiple disciplines to ensure a seamless experience for patients.
• Follow physician and nursing orders diligently.
• Assess and coordinate discharge planning with the healthcare team.
• Engage in rounds with the physician when necessary in an inpatient environment.
• Utilize professional nursing standards and the ANA Code of Ethics in all interactions.
• Maintain a safe and alert working environment.
• Perform additional duties as assigned.

Requirements :

Education : Graduation from an accredited nursing school.

Experience : No prior experience is necessary; new graduates are welcome!

Physical Requirements :

Licensure :

Registered Nurse license is required in the state where you will work. Basic Life Support (BLS) certification is also essential.

This position requires a day shift commitment.
