Key Responsibilities
Conduct member assessments and coordinate care plans with various departments
Facilitate referrals to providers and track engagement to ensure timely access to care
Serve as the main contact for members, providing follow-up coordination and support
Required Qualifications
2+ years of experience in healthcare coordination, case management, or member services
Proficiency with CRM systems and healthcare databases
Familiarity with Medi-Cal, CalAIM, and HIPAA standards preferred
Strong organizational skills and ability to work in high-volume environments
