Do you believe that home is where the heart (and the best care) is? We are looking for a high-energy, empathetic Health Coach to join our team and support our Structured Family Caregiving program.
In this role, you aren’t just a coach; you’re a champion for independence, dignity, and family-centered care.
You’ll help patients stay in their own homes by supporting the incredible caregivers who look after them every day.
The Mission
You will be the bridge between our waiver members, their caregivers, and the clinical team.
Whether you’re visiting a home to ensure safety or reviewing daily notes to spot where a caregiver needs extra support, you are the "eyes and ears" that ensure our clients thrive.
