Role Scope :

The Clinical Services Manager, reporting to the Executive Director, focuses on the management of the Habitat Health clinic, rehabilitation services, social work, and behavioral health services. This includes providing ongoing guidance and support to the Interdisciplinary Team (IDT) as a member of the leadership team.

Core Responsibilities & Expectations for the Role

• Create a center experience that Participants want to spend time in, and team members love. Establish a team that cares and creates joy, an environment where all participants and team members belong, and lead a service-oriented operation.
• Continue to raise the bar, including leading the clinical quality improvement process. Constructively seek and share feedback, and quickly implement changes in order to improve how the center and the PACE program as a whole achieve their goals and mission.
• Establish and honor Habitat values within the team.
• Accountable for the delivery of high-quality clinic, social work and behavioral health services.
• Provide ongoing clinical guidance and support to the IDT, and provide back-up planning and facilitation of the IDT meetings as needed.
• Direct clinic and social work operations including hiring, training, supervising, and mentoring team members and ensuring clear and effective workflows
• Ensure participants experience smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.), deploy team members as needed, and work collaboratively with the clinical team, Operations Manager, and Medical Director
• Achieve key performance indicators as outlined in monthly operating reviewing and weekly tactical meetings
• Maintain compliance with PACE, ADHC, and DHCS regulations inclusive of audit prep, monitoring non-compliance, and implementation of corrective action plans
• Performs related duties as assigned, including, but not limited to filling gaps in direct participant care.

Compensation :

We take into account an individual's qualifications, skillset, and experience in determining final salary. This role is eligible for medical / dental / vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $130,000 - $160,000. The actual offer will be at the company's sole discretion and determined by relevant business considerations, including the final candidate's qualifications, years of experience, skillset, and geographic location.
