Portage, with its welcoming community and convenient location, provides an excellent environment for a physician dedicated to providing comprehensive care to patients of all ages. Our clinic is committed to delivering high-quality primary care services to patients in the Portage area.

Salary and Benefits:

• Salary Range: $230,000 to $250,000 annually
• Vacation: 160 hours
• Personal Time: 40 hours
• Continuing Medical Education (CME): 40 hours, allowance of $2,500
• Medical, Dental, and Vision Insurance
• Life Insurance: 1.5x the provider’s salary
• Short-Term and Long-Term Disability
• 401k Plan: matches up to 4%
• FTCA Malpractice Insurance

Responsibilities:

• Provide comprehensive primary care services to patients of all ages.
• Schedules such as 5-8s, 4-10s, ensuring Work / Life Balance.
• No On-Call, No Nights, No Weekends.
• Full Support Staff to aid in patient care and clinic operations.
• Collaborate with a multidisciplinary team to ensure high-quality care for patients.

Requirements:

• Board-certified or Board-eligible Family Medicine Physician
• Compassionate, patient-centered approach in primary care
• Clear and Active Medical License in the State of Indiana
