Job Requirements

Education Requirements

Degree in Physical Therapy (DPT or equivalent) from an accredited program

Certifications Requirements

Active California State Physical Therapist license

CPR certification

Experience Requirements

Minimum 1 year of home health experience

Proven ability to manage 5-6 patient visits daily

Other Requirements

Must have at least 1 year of home health experience

Strong communication and compassionate patient care skills

Ability to work independently and adhere to treatment plans

Must pass background check and provide references

Physical stamina to manage 5-6 visits per day

Develop personalized treatment plans and adjust based on patient progress

Manage 5-6 patient visits per day within a 30-mile radius of Jackson, CA

Educate patients and families on mobility, strength, and pain management

Document patient progress accurately in the EMR system (Matrix)
