is hiring a full-time
Nurse Practitioner (NP)
or
Physician Assistant (PA)
to join its team in delivering high-quality, community-based family medicine. This position is ideal for a provider seeking

Compensation & Benefits
Salary Range :
$100,000 ? $140,000 annually (based on experience)
Bonus Plan :
Percentage-based incentive linked to collections

Key Responsibilities
Perform physical exams, preventive care screenings, and wellness visits
Diagnose and treat minor injuries and acute illnesses
Manage chronic conditions such as diabetes, hypertension, and COPD
Order and interpret labs, imaging, and diagnostic tests
Counsel patients on health education, lifestyle changes, and treatment adherence
Coordinate referrals and ensure follow-up care is completed
Maintain accurate records within the electronic health record (EHR) system
Contribute to internal quality initiatives and peer review activities
Qualifications
Active and unrestricted Arizona license as an NP or PA
Graduate of a U.S.-based accredited Nurse Practitioner or Physician Assistant program
(Note : Programs based in Puerto Rico are not accepted)
DEA certification (or eligibility prior to start)
Able to credential with Medicare, Medicaid, and major commercial insurance plans
No history of malpractice or disciplinary actions
Preferred :
At least one year of experience in outpatient primary care or family medicine
