As a Care Manager Registered Nurse, you will play a vital role in advancing patients through their care journey in the acute care setting. Your key responsibilities will include assessing post-acute needs, facilitating transition plans, coordinating care, providing education, making community care referrals, and collaborating with the care team to ensure effective care management. You will also be involved in the ongoing monitoring and reassessment of care plans to meet regulatory standards and improve patient outcomes.

Required Qualifications :

• Nursing degree or diploma required on hire.
• Bachelor's degree in Nursing or higher must be obtained within 3 years of hire.
• Oregon Registered Nurse License required on hire.
• Minimum of 2 years of experience in an acute care hospital or community setting.
• At least 1 year of experience with electronic health records or documentation systems.
• 1 year of experience in home health, mental health, substance use, hospice, utilization review, discharge planning, or care management.
