Job Summary

Provides direct and indirect counseling services to patients and their families using individual, group, and family therapy. Provides bio-psychosocial assessments, risk assessments, treatment planning and coordination with multidisciplinary team. Remains open to clinical and peer supervision process.

Essential Functions

• Provides direct and indirect clinical / therapy services to patients and families covering a broad range of standard practice modalities and theories, biopsychosocial assessments, diagnostic determinations, individual, group, and family therapy, aftercare services.
• Participates in the planning, development, and implementation of the therapeutic program, including strategies for quality improvement, patient satisfaction, regulatory adherence, and associated policies, processes, chart audits.
• Develops an individualized treatment plan including measurable treatment goals related to patient's presenting problems. Participates as an active member of the multi-disciplinary treatment team.
• Performs case management responsibilities involved with patient care. This includes, but is not limited to, Utilization Review, Discharge Planning, Family Involvement / Education, timely and appropriate documentation as needed.
• Serves as team lead and operates as a resource to staff. Supervises graduate students as field instructor / site supervisor.
• Performs other duties as assigned.
