What You'll Do

Provide one-on-one home-based physical therapy

Complete evaluations and build individualized treatment plans

Educate patients and caregivers on mobility and safety

Collaborate with nursing and therapy partners

Travel within the Salem service area for home visits

Qualifications

Active PT license or eligibility for Oregon licensure

Doctorate or Master's in PT

1+ year of PT experience (home health preferred)

CPR / BLS certification

Reliable transportation
