HealthCare Support is actively seeking a Nurse Practitioner to fill an opening with Managed Care partnered SNF in Fairview Manor, PA .

Daily Responsibilities for Nurse Practitioner :

• Conduct patient facing comprehensive assessments between 4 SNF facilities to prevent unnecessary hospital readmissions for acute and chronic conditions
• Conduct advanced illness and advanced care planning conversations for 4-5 patients / day
• Mon - Fri 8am-5pm
• 7-8 Month Right to Hire contract for understaffed region.

FTE Conversions available as early as 3 months.
