VITAE - Nurse Practitioner Program

The Position

As we continue to grow, we are looking for “SNF-ists,” in-house n urse practitioners that will provide care for the geriatric population in a Skilled Nursing and / or Assisted Living setting.

What You Will Do

As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services. You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility’s residents.

You will work with the entire facility’s patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.

As a SNF-ist, you will provide types of care, including but not limited to :

• Be comfortable and proficient with documentation and CPT coding
• Evaluation of new-admission visits
• Coordination of discharge visits
• Monthly stability visits for long term residents
• Evaluations for patients exhibiting changes in condition
• Advanced Care Planning
