A company is looking for a Health Navigator to join their Member Outreach team.
Key Responsibilities
Conduct educational calls to advise members on benefits and services, complete health needs assessments, and refer to population health programs
Reach out to members with gaps in care, encouraging compliance and assisting with provider and appointment scheduling
Manage system work queues, screening members for eligibility and assigning cases to the clinical team for outreach
Required Qualifications
Associate's Degree in education, communication, health-related field, or equivalent work experience
2 years of experience in a customer service support role
Ability to work independently with minimal supervision
Knowledge of medical terminology
Digital literacy assessment required
