Job Description

Job Title : Physical Therapist

Experience : Minimum 1 full year of adult home health experience

License : Must have a valid license in hand

Certifications : CPR, BLS

Must-Have : Recent home health experience

Description :

Home visits for patients.

Periodic office meetings required.

Must have reliable transportation.

Must possess a current driver's license and agency-required liability insurance.

Average number of patients per day is 5-6.

Contractor will be assigned between 5-10 patients daily.

Each type of visit and activity carries a different point value.

Contractor is expected to meet productivity points of 30 points per week.

Must be willing to travel up to 75 miles from the home clinic.

Mileage will be reimbursed at the IRS standard rate.

Will visit patients in home settings and assisted living facilities.

Weekend coverage requirements vary based on location and profession.

Contractors must accept every patient scheduled regardless of location.

All documentation must be completed within 24 hours of the appointment.

There are no guaranteed hours with this contract.

Delayed and incomplete documentation may result in cancellation.

If low patient census affects financial needs, discussions for floating to a nearby clinic may be necessary.
