Description : Work Schedule :
100% FTE, day shift. Monday through Friday between the hours are 8 : 00 AM – 4 : 30 PM. Hours may vary depending on the operational needs of the department. You will primarily work at the Johnson Creek Cancer Center with occasional travel to UW Health’s surrounding cancer clinics in the Region. You will work primarily on site (at Johnson Creek) and pending operational capabilities may be able to work one day a week remote from home.

We are seeking an RN Oncology Navigator to :
Be a leader in providing comprehensive cancer care for patients and caregivers throughout all phases of cancer care.
Assess patients’ barriers to care, psychosocial, and learning needs to advocate for and support patients and caregivers, collaboratively with teams and support services, as we strive to provide equitable care and outcomes for our patients.
Facilitate timely and coordinated transitions of care, provide anticipatory guidance, reinforce patient and caregiver education, provide guidance in the shared decision-making process, and provide mental and emotional support to patients and caregivers through the continuum of cancer care.
Participate in multidisciplinary tumor board meetings.
Participate in continuous quality and process improvement initiatives.
