Wisconsin Licensed RN Care Manager

A company is looking for a RN Care Manager with a Condition / Disease Management focus.

Key Responsibilities

Provide case management services including assessment, planning, facilitation, care coordination, and evaluation

Review medical and behavioral test results, update care plans, and close cases when goals are achieved

Educate members and families about case management, health conditions, and available resources

Required Qualifications

Graduation from an accredited school of nursing

Bachelor's degree in Nursing preferred

RN licensure in the State of Wisconsin

Four years of clinical health care experience as a RN required

Case Management certification preferred
