Physical Therapist - Home Health

Acclaim Home Health is seeking a dedicated and compassionate Physical Therapist to join our Home Health team in Worcester, MA. This role involves providing high-quality physical therapy services to patients in their homes, helping them recover and improve their physical function in a comfortable and supportive environment.

Responsibilities

• Evaluate patients' physical conditions and develop individualized treatment plans.
• Provide skilled physical therapy services in patients’ homes, focusing on rehabilitation, mobility improvement, and pain management.
• Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care.
• Document patient evaluations, treatments, and progress notes thoroughly and accurately.
• Educate patients and their families on exercises and activities to promote recovery and avoid injury.

Requirements

• Licensed Physical Therapist in the state of Massachusetts.
• Experience in home health or outpatient physical therapy preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Valid driver’s license and reliable transportation to visit patients’ homes.
• Current CPR certification.
