Job Title : Nurse Practitioner

Job Description :
Conduct 10 12 patient visits daily across 3 8 DaVita dialysis centres in the Chesapeake / Norfolk area.
Perform comprehensive health evaluations and manage chronic conditions such as diabetes hypertension COPD and heart disease.
Prescribe titrate and monitor medications for complex chronic conditions.
Coordinate patient care with nephrologists dialysis teammates and primary care providers.
Lead care innovation providing feedback to DaVitas leadership team on new models of care.
