    • Why does the job exist?

    The Social Services Coordinator will :

  • Represent the interests of persons with Developmental Disabilities and Medically Fragile conditions in the implementation of the Mi Via Self Directed waiver;
  • Work closely with state partners in the Health Care Authority department and other bureaus within the Developmental Disabilities Supports Division.

    • How does it get done?

    The Social Services Coordinator is responsible for :

  • Coordinating access in participant services;
  • Providing information;
  • Training;
  • Implementing policies, procedures and service standards;
  • Monitoring assurances for Mi Via waiver implementation;
  • Providing oversight and technical assistance to Consultant Agency providers on a statewide basis.

    • Who are the customers?

  • Individuals served by the Health Care Authority, Community Based Waivers and the Mi Via Self-Directed Waiver Program

    • Ideal Candidate

    The ideal candidate has :

  • Bachelor's Degree in Social Work, Psychology, Education, Sociology and Family Studies;
  • Four (4) years of experience in supportive employment, vocational rehab, social work or case management;
  • have experience with assistance to Case Mangers, Providers, and Therapists regarding the Regulations DDW Service Standards, and Policies and Procedures for the Developmental Disabilities Waiver and State General Fund programs;
  • will be adept to prioritizing numerous competing tight deadlines and will have strong organizational skills;
  • will have a strong work ethic, employ good critical thinking and communication skills.

    • Minimum Qualification

    Associate's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology, Family Studies / Services, Social Sciences, or Human Services and one (1) year of experience as a social work assistant and / or in social or community coordination, working with communities, working on health or social service related matters, social work / case management experience, and / or behavioral health care. Any combination of education from an accredited college or university in a related field and / or direct experience in this occupation totaling three (3) years may substitute for the required education and experience.

    Employment Requirements

    Must possess and maintain a valid New Mexico Driver's License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive a Defensive Driving Course Certificate as a condition of continued employment within 45 days of employment. Travel may be required with some occasional overnight stays. Successful completion of the Health Insurance Portability and Accountability Act (HIPAA).

    Working Conditions

    Office setting with extensive use of personal computer, telephone, fax, copier and other related office equipment that involves sitting, standing, bending, reaching, etc. to include lifting up to 30 pounds.
