Responsibilities :

Provide direct patient care to residents, including administering medications, wound care, and assisting with daily living activities

Perform vital signs, monitor health status, and report any changes to the medical team

Develop and implement individualized care plans in collaboration with the interdisciplinary team

Assist with admissions, transfers, and discharges, ensuring a smooth transition for residents

Maintain accurate and up-to-date records, including charts, reports, and logs

Participate in quality improvement initiatives and maintain compliance with regulatory standards

Collaborate with other healthcare professionals to ensure comprehensive care

Requirements :

Current LPN license in the state of Nebraska

Strong communication and interpersonal skills

Ability to work effectively in a fast-paced environment

Commitment to providing person-centered care and promoting resident autonomy
