Job Overview
We are seeking an energetic and detail-oriented LPN Healthcare Manager to lead and coordinate nursing services within our community support programs. This vital role involves overseeing patient care, managing clinical staff, ensuring compliance with healthcare regulations, and fostering a compassionate environment for individuals with complex health needs. The ideal candidate will bring critical care experience, strong leadership skills, and a passion for enhancing quality of life for those we serve.
Duties
- Supervise nursing staff to deliver high-quality patient care across various settings, including residential and community-based programs
- Oversee clinical assessments, vital signs monitoring, medication administration, and medical documentation in accordance with HIPAA standards
- Coordinate medical procedures such as IV infusions, catheterizations, tube feedings, and sterile techniques while ensuring safety protocols are followed
- Manage EMR (Electronic Medical Records) systems like Epic or eClinicalWorks to maintain accurate patient records and documentation
- Conduct patient assessments, triage calls, and coordinate urgent or emergent care needs in collaboration with healthcare providers
- Develop discharge plans and collaborate with case management teams to ensure comprehensive care continuity
- Provide training and mentorship to staff on topics such as infection control, HIPAA compliance, ICD coding (International Classification of Diseases), and other clinical best practices
Qualifications
Valid LPN license with current state licensure required; RN licensure considered a plusExtensive experience in critical care environments such as ICU or emergency medicine preferredProven background in medical office management, long-term care, assisted living, or nursing home settingsHands-on experience with EMR / EHR systems including Epic, Cerner, or Athenahealth; familiarity with ICD-9 / 10 coding highly desirableStrong knowledge of physiology, anatomy, pharmacology, and medical terminologyExperience administering injections, phlebotomy, ventilator management, and wound suturing preferredDemonstrated ability to lead multidisciplinary teams while maintaining compliance with healthcare regulations such as HIPAA and NCQA standardsExcellent patient assessment skills combined with compassionate communication abilities for working with people with disabilities or complex health conditionsJoin us to make a meaningful difference in the lives of individuals with DD / ID by providing exceptional nursing leadership within a supportive community environment.
