Geriatric Nurse for Adult Day Health RN/LPN

LPN/RN will work closely Adult Day Team. This job requires excellent communication and organizational skills, must have an active GA LPN/RN license. Must enjoy working with the elderly and disabled. Must have good documentation skills is a must. Must be proficient in Microsoft office. Medication administration and assessments required.

Job Duties

o Collaborate with healthcare providers, case management Agencies and Area on Aging

o Daily assessments of clients. Monitor clients for signs and symptoms.
o Document daily census and nursing assessments.

o Communicate with clients, families and staff.

o Intial assessments of new ADH clients.

o Monthly assessments
o Facilitate the exchange of medical information between different healthcare providers as needed

o This is not a hybrid position. Position is at the Adult Day Center.
