You will...

Educate patients and their families on proper care techniques, medication administration, and health promotion strategies.

Communicate effectively with patients, families, and healthcare providers to ensure continuity of care and positive outcomes.

Participate in ongoing training and professional development opportunities to enhance your skills and knowledge.

Provide one-on-one skilled nursing care and treatment to patients in their home.

Assist the registered nurse in assessment of patient's condition and contribute information to aid in planning and evaluating care.

Demonstrate knowledge of home health and competencies in discipline-specific patient care skills required by the care center to provide patient care.

Check patient's vital signs, Perform wound care, and educate patients and their families about their medications and chronic conditions.

Provide patient episode management per protocol.

Complete documentation timely and accurately according to industry standards, conventions, and guidelines, including subsequent visits, or other required documentation.

You have...

Strong assessment, critical thinking, and communication skills.

Ability to work independently and as part of a dynamic team.

Comfortability using technical tools and software / EMR for visit documentation

Current CPR certification.

Reliable transportation, current driver's license, and car insurance coverage.
