Massachusetts Licensed RN Case Manager

A company is looking for an RN Case Manager to facilitate care for members with complex healthcare needs.

Key Responsibilities :

Engage members in care plans, coordinate services, and promote compliance with treatment goals

Interpret case management criteria to document comprehensive care management plans

Connect with treatment providers to gather clinical information and monitor quality of care

Key Qualifications :

3-5 years of relevant clinical healthcare experience

Active licensure in Massachusetts; additional state licensure is a plus

Utilization Management experience preferred

Proficient with multiple IT systems

Ability to work effectively with diverse populations
