Located in beautiful Louisville, CO, our 104-bed facility provides a peaceful setting for those seeking treatment for mental illness including chemical dependency. Centennial Peaks offers unique and individualized programming for adolescents and adults that sets us apart from many other treatment facilities, and our dynamic leadership team includes a seasoned nursing department, ECT Therapy, Intensive Outpatient and Partial Hospitalization Programs.

The Social Worker provides clinical social work services on units as assigned in a manner consistent with the philosophy and objectives of Centennial Peaks Hospital. The Social Worker is responsible for psychosocial assessments, individual, group and family therapy, participation in treatment team planning, processing commitments, and discharge planning / case management service, crisis intervention and liaison services with referral sources and other departments within the hospital.
This position is eligible for our generous shift differentials : Weekend Day (Sat-Sun) $2 / hr, Evening (Mon-Thurs) $3.25 / hr, Weekend Evening (Fri-Sun) $5.00 / hr, Night (Mon-Thurs) $5.00 / hr, Weekend Night (Fri-Sun) $7.00 / hr.
PRN Shift Differential : $1.50 / hr
