[job_card.job_description]Flexible Schedule Created by You Paid per hour (not just per visit) No weekends, evenings, or holidays Drive Time Reimbursed at Clinical Rate & Mileage Reimbursement Collaborative Team Environment Educational Programs Growth / Advancement Opportunities Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families Sets measurable objectives that are formulated in conjunction with the rehab team Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems Instructs / counsels' patients and families in treating and coping with social and emotional response connected with illnesses Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs Identifies and assists the patient and / or family to utilize appropriate community resources to achieve measurable objectives Master's Degree in Clinical Social Work (LCSW) Current Licensed Clinical Social Worker Licensure in the state of practice Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis Current BLS (Basic Life Support) First Aid and CPR certification required Ability to lift 50 pounds Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures Duties require fine motor skills, visual acuity, and walking / standing for extended periods Additional physical requirements include pushing / pulling, bending / stooping, reaching, kneeling, and positioning frequently at times Focus on community reintegration and functional outcomes - transforming everyday activities like grocery shopping, hiking, or even paddle boarding into meaningful therapy experiences. Full-Time clinicians enjoy a guaranteed salary , while PRN clinicians benefit from flexible scheduling and hourly pay for time worked - including direct therapy & indirect services, like documentation, communication, and care coordination. Each client typically receives 4-6 hours of therapy per discipline / per week, allowing schedules to be tailored to meet the needs of both clinician and client. Enjoy co-treatment opportunities across disciplines - PT, OT, SLP, and Social Work - fostering a truly collaborative, team-based approach. Focus on providing 1 : 1 individualized therapy while our Clinical Coordinators handle prescriptions, authorizations, and other administrative tasks, so you can spend your time delivering exceptional care that drives real-world results.

Who we are looking for :
We are seeking a Licensed Clinical Social Worker (LCSW) to provide specialized mental health counseling for social and emotional adjustment during rehabilitation from catastrophic injury or illness.
- This position involves supporting individuals and families as they adapt to disability and lifestyle changes.
- You will provide therapeutic interventions, as well as resource support and navigation that address the patient's goals.
- Knowledge of medical social work and / or neurological injury (TBI / CVA)
