Overview :
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our interdisciplinary care team. This role provides nursing care and clinical support to individuals with intellectual and developmental disabilities (IDD) and those with complex medical, mental health, and behavioral health needs . The LVN plays a key role in promoting the health, safety, and well-being of residents while supporting their independence and quality of life.
Key Responsibilities :
Provide direct nursing care, including administering medications, monitoring vital signs, wound care, and assisting with daily living activities as needed.
Observe and document changes in residents’ physical or mental condition, reporting concerns promptly to the Registered Nurse (RN) or Physician.
Implement individualized care plans in collaboration with the multidisciplinary team, including physicians, therapists, case managers, and behavioral specialists.
Support behavioral intervention plans and ensure trauma-informed, person-centered approaches are maintained.
Maintain accurate medical records in compliance with facility policies and state regulations.
Provide health education and support to residents, families, and direct care staff regarding medication, hygiene, and preventive care.
Participate in staff training, team meetings, and emergency response procedures.
Ensure infection control and safety standards are consistently upheld.
Qualifications :
Current and active LVN license in the state of [Insert State].
Minimum of 1–2 years of nursing experience , preferably in developmental disabilities, behavioral health, or long-term care settings.
Knowledge of intellectual and developmental disabilities (IDD) , behavioral intervention strategies, and trauma-informed care.
Strong communication, teamwork, and problem-solving skills.
Ability to handle challenging situations with patience, empathy, and professionalism.
CPR and First Aid certification (or willingness to obtain).
Work Environment :
Facility-based setting providing 24-hour care and support.
Collaboration with multidisciplinary teams to ensure holistic care.
May require standing, lifting, or assisting residents with mobility and personal care needs.
