The YHDP Navigator guides youth through systems of care, connects them to resources based on their needs and choices, and creates individualized care plans. The YHDP Navigator helps prevent youth from entering homelessness or supports them in quickly moving into safe housing. The role provides immediate response, mobile case management, personalized safety and service planning, and referrals to community services. The YHDP Navigator also builds a comprehensive resource guide and strengthens partnerships with local organizations to improve access for youth.KEY RESPONSIBILITIES : Use housing problem-solving to help youth find safe, empowering alternatives to homelessness or secure immediate shelter when needed.Educate youth about homelessness and available local services, ensuring quick access to supports.Track and report client information in the HMIS system.Conduct outreach to youth-serving institutions and communities to identify eligible youth.Work with the Youth Coordinated Entry System Coordinator to match youth with transitional or permanent housing, complete assessments, and participate in case conferences.Support youth in creating and achieving short- and long-term goals using evidence-based practices (e.g., motivational interviewing, positive youth development, trauma-informed care).Screen and assess participants needs using tools like VI-TAY and connect them to resources, referrals, and shelters as appropriate.Attend trainings and conferences to build knowledge, especially on domestic violence, sexual exploitation, and human trafficking, and provide safety planning and rapid linkages to specialized services.Participate in relevant meetings, including Youth System Navigation team and youth housing prioritization meetings.
