Guided by our core values of integrity, community, diversity, innovation, and passion, we have built a team of mission-aligned people who are inspired by social justice and fueled by a deep commitment to excellence, with joy.POSITION SUMMARYThe Homeless Youth Systems Navigator (also referred to as the YHDP Navigator) helps unaccompanied and / or pregnant / parenting youth ages 18-24 who are experiencing homelessness or seeking support. The YHDP Navigator guides youth through systems of care, connects them to resources based on their needs and choices, and creates individualized care plans. The YHDP Navigator helps prevent youth from entering homelessness or supports them in quickly moving into safe housing. The role provides immediate response, mobile case management, personalized safety and service planning, and referrals to community services. The YHDP Navigator also builds a comprehensive resource guide and strengthens partnerships with local organizations to improve access for youth.KEY RESPONSIBILITIES :  Use housing problem-solving to help youth find safe, empowering alternatives to homelessness or secure immediate shelter when needed.Educate youth about homelessness and available local services, ensuring quick access to supports.Track and report client information in the HMIS system.Conduct outreach to youth-serving institutions and communities to identify eligible youth.Work with the Youth Coordinated Entry System Coordinator to match youth with transitional or permanent housing, complete assessments, and participate in case conferences.Support youth in creating and achieving short- and long-term goals using evidence-based practices (e.g., motivational interviewing, positive youth development, trauma-informed care).Screen and assess participants needs using tools like VI-TAY and connect them to resources, referrals, and shelters as appropriate.Attend trainings and conferences to build knowledge, especially on domestic violence, sexual exploitation, and human trafficking, and provide safety planning and rapid linkages to specialized services.Participate in relevant meetings, including Youth System Navigation team and youth housing prioritization meetings. PREFERRED QUALIFICATIONS :  Bachelors degree in human services, public health, or a related field (Masters preferred), or at least 2 years of relevant experience and training in social work, case management, advocacy and / or care coordination.Experience providing case management or support to youth preferredStrong commitment to serving under-resourced communities and a passion for addressing issues such as homelessness, domestic violence, substance abuse, and mental illness.Strong knowledge of local resources and the ability to collaborate effectively with community partner agencies to connect clients to services.Preferred : extensive knowledge or lived / working experience in systems such as foster care / DCS, juvenile justice, domestic violence, HIV care, healthcare / mental health, LGBTQ+ services, DHS benefits (SOAR / SNAP / TANF), immigration, or substance abuseSkilled in MS Word, Excel, Outlook, internet use, and other necessary software.Proficient computer and research skills, with the ability to find, analyze, and interpret data related to homelessness and similar issues.Experience with federal grants and familiarity with federal regulations and guidelinesBilingual in English and Spanish preferred, but not requiredESSENTIAL COMPETENCIES : Knowledge of secondary transition, work experience programs, relevant laws, and district policies.Ability to build and maintain positive relationships with employers, supervisors, and community partners.Willingness to learn, adapt, and thrive in changing environments.Passionate and sensitive to the needs of youth, adults, and families.Strong teamwork, leadership, and organizational skills.Excellent communication and interpersonal skills with diverse populations.Ability to manage multiple tasks in a fast-paced environment.Comfortable using data and metrics to track and report program success.Demonstrates integrity, initiative, professionalism, and ethical behavior.Innovative, strategic, and goal-oriented problem-solving.Flexible, adaptable, and able to respond to evolving program needs.Detail-oriented with strong follow-up and follow-through.Committed to diversity, equity, and inclusion, and fostering a respectful, harassment-free workplace.SPECIAL REQUIREMENTS : Must have a valid drivers license, proof of automobile insurance, and reliable transportation.COMPENSATION AND BENEFITS : Compensation is commensurate with experience and includes a comprehensive, competitive benefits package.Salary Range for the position : $49,000-$52,000Health Care Plan (Medical, Dental & Vision)Long-Term and Short-Term Disability Retirement Plan (401k)Life Insurance (Basic, Voluntary & AD&D)Paid Time Off (Vacation, Sick & Public Holidays)HOURS : This is a full-time position with standard hours of 8 : 30 a.m. to 5 : 00 p.m. However, the role requires flexibility and may occasionally involve evening or weekend hours to support activities and meet reporting deadlines.Target Start Date :  January 5, 2026IMPORTANT NOTICE : To be considered for employment and advance to the next stage of the hiring process, you must complete the following assessment.
