Position Summary: The On-Call Hospice RN is responsible for providing skilled nursing care and support to hospice patients and their families during after-hours, weekends, and holidays. This role ensures patients receive timely and compassionate care by responding to calls, triaging symptoms, and making visits as needed during the on-call shift.
Key Responsibilities: - Respond promptly to after-hours calls from patients, families, or facilities.
- Provide phone triage, assess symptoms, and determine the appropriate level of intervention.
- Make home or facility visits as necessary to provide hands-on nursing care.
- Communicate changes in patient condition to the interdisciplinary team.
- Provide emotional support and guidance to patients and families during crisis situations.
- Document all visits, calls, and care provided in accordance with agency policy and hospice regulations.
- Collaborate with the on-call manager and case managers to ensure continuity of care.
- Participate in on-call rotation and remain available during assigned shifts.
- Educate families and caregivers on medication administration, symptom management, and the dying process.
- Follow all hospice policies, procedures, and safety guidelines.
Qualifications: - Current and unrestricted Registered Nurse (RN) license in the practicing state.
- Minimum of 1 year of recent hospice, palliative, or home health experience preferred.
- Excellent clinical assessment and critical thinking skills.
- Strong communication, interpersonal, and organizational skills.
- Ability to work independently in a home or facility setting.
- Valid driver's license, reliable transportation, and proof of auto insurance.
- Proficient in electronic medical record (EMR) systems and documentation.
Work Schedule: - This is an on-call position covering evenings, nights, weekends, and/or holidays as assigned. May include 12- or 24-hour shifts depending on agency policy.
Work Environment: - Patient homes, skilled nursing facilities, or assisted living centers.
- Requires travel within a defined service area.
