Outreach Specialist

The Excalibur Group is currently seeking an Outreach Specialist to join our dynamic team in Washington D.C.

DUTIES / RESPONSIBILITIES :

• Outreach Specialists shall be responsible for initiating contact with assigned Customers and encouraging Customers to engage with the Provider and participate in mandated activities.
• Customers may require repeated attempts at engagement.
• Outreach Specialists shall also be responsible for re-engaging those Customers who were engaged but who subsequently stopped participating.
• Outreach Specialists shall actively share information about outreach and engagement efforts with Case Managers.
• Outreach Specialists shall also inform Case Managers of any presenting barriers that prevent the Customer from fully engaging with the Provider.
