Responsibilities

What You'll Do :

The Licensed Practical Nurse (LPN) is vital in delivering comprehensive care to both youth and adults, focusing on the administration and documentation of medications, transcribing provider orders, and performing continuous assessments. The role encompasses gathering detailed data on behavioral and physical health, including substance use history, and accurately documenting observed behaviors and interventions. Additionally, the LPN is responsible for educating individuals and their families about behavioral health diagnoses and medications during both admission and discharge, aiming to enhance treatment adherence and overall care outcomes.

• Nursing Support and Collaboration :
• Contribute data for the Nursing Assessment, ongoing re-assessment, and health history under the direct supervision of an RN.
• Collaborate with the RN on the individual’s care plan, reporting pertinent information such as blood glucose levels, CIWA / CINA scores, and breathalyzer results.
