Physical Therapist (Skilled Nursing Facility)

Location: Height Street Skilled Center, Bakersfield, CA
Job Type: Full-time | Part-time | Per-Diem

We are seeking a dedicated and patient-focused Physical Therapist (PT) to join our rehabilitation team at our Skilled Nursing Facility. As a Physical Therapist, you will play a critical role in helping our residents improve mobility, reduce pain, and regain independence after illness, injury, or surgery. You'll be part of a multidisciplinary team committed to providing the highest standard of care in a supportive and collaborative environment.
Responsibilities

• Evaluate and assess patients to develop individualized treatment plans
• Provide physical therapy services to patients recovering from injury, illness, or surgery
• Collaborate with physicians, nursing staff, and other healthcare professionals to coordinate patient care
• Monitor patient progress and adjust treatment plans as necessary
• Educate patients, families, and caregivers on therapy techniques and home exercises
• Maintain accurate and timely documentation in accordance with state, federal, and facility guidelines
• Ensure compliance with all regulatory and facility standards
• Contribute to team meetings and care planning for residents

Qualifications

• Graduate of an accredited Physical Therapy program
• Current and valid Physical Therapist license in the State of California
• CPR certification (current and valid)
• Experience in a skilled nursing or long-term care setting preferred, but not required
• Strong communication and documentation skills
• Ability to motivate and encourage patients in a compassionate, respectful manner
