job_description.job_card.job_descriptionFlexible Hours- Choose your Days No Evenings, Weekends, or Holidays Clinical Autonomy Supportive Team and Leadership Professional Growth & Education Opportunities Perform evaluations and develop treatment plans Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources Communicate patient's needs and progress to the treatment team, physician, person receiving services and family members Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes Minimum of a bachelor's degree in Occupational Therapy from a college or university with an accredited Occupational Therapy program Current, unrestricted license as an Occupational Therapist by state in which practicing Current CPR Certification A minimum of one year's work experience as an Occupational Therapist Demonstrates knowledge of rehabilitation techniques related to complex neurological diagnoses preferred Communicates effectively and professionally in verbal and written interaction Ability to lift 50 pounds Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures Duties require fine motor skills , visual acuity, and walking / standing for extended periods Additional physical requirements include : pushing / pulling, bending / stooping, reaching, kneeling, and positioning frequently at times SWAN Rehab is staffed by physical, occupational, and speech therapists as well as vocational specialists including neuropsychologists and social workers trained in neurological rehabilitation SWAN Rehab works with Northern Arizona University and AT Still University, and Arizona State University for clinical research Each treatment session is a full hour of one-on-one therapy with a therapist, not a technician Therapy provided is intense and tailored to the individual in a supportive enriched environment SWAN employs many of the latest technologies and clinical techniques for neurological rehabilitation Each patient is discharged with a personalized written home program to maintain progress made in therapy and to continue improvement in function SWAN offers a community exercise program, called HOPE, for discharged and uninsured patients SWAN Rehab is one of four exclusive programs in Arizona that provides Comprehensive Neurological Vocational Rehab for the Department of Economic Security SWAN offers specialized programs in Vestibular Rehabilitation and Post-Amputation Rehabilitation

Who we are looking for :
Provide 1 : 1 patient care in 60-minute treatment sessions , focusing on functional rehabilitation for adults with neurological injuries
Collaborate closely with an interdisciplinary team of passionate, skilled therapists (PT / OT / SLP / COTA / PTA) to focus on patient-centered outcomes
Provide personalized care in an innovative outpatient setting utilizing the latest technology and equipment including Bioness BITS, FES, biVABA, Assistive Technology, and more!
