Schedule : Monday thru Friday 9am to 5pm
Salary : $20 to $22 / hr
General Description
The Health Case Manager provides comprehensive health case management for refugee clients identified as having special health needs. Working collaboratively with departmental colleagues, this role ensures that vulnerable clients have access to both mental health care and specialized services available in the community. The Health Case Manager fosters consistent, collaborative communication across program staff and maintains a trauma-informed care environment to support client wellbeing.
Essential Duties and Responsibilities
- Maintain a caseload of clients and ensure all case files are complete and compliant with agency, departmental, regulatory, and funder guidelines.
- Conduct relevant screenings and assessments, including but not limited to psychosocial, mental health, financial, and employment status.
- Develop, implement, and monitor comprehensive, solution-focused service plans with measurable goals.
- Assist in discharge planning to ensure safe and effective transitions.
- Facilitates, transports and accompanies clients to relevant appointments, activities, or meetings as needed.
- Support clients in understanding and meeting the expectations of service providers.
- Connect clients with opportunities for social support, such as recreational activities, faith-based groups, or peer-support programs.
- Provide educational guidance on topics that promote client self-sufficiency.
- Collaborate with referring agencies, funders, and internal staff to effectively address client needs.
- Maintain regular communication with service providers to coordinate benefits and address treatment needs.
- Report urgent client concerns or incidents promptly and in accordance with established protocols.
- Advocate for clients by completing letters of request, referral forms, and eligibility documentation for charitable organizations and public agencies.
- Maintain current knowledge of community resources and connect clients to appropriate opportunities and services.
- Document all case activity in agency-approved databases or online platforms in compliance with regulatory standards.
- Meet or exceed productivity and documentation standards to support the agency’s financial stability.
- Notify supervisor of significant client-related events and follow the program’s incident reporting procedures.
- Participate in program, departmental, and agency-wide meetings, case reviews, and training sessions.
- Engage in professional development through continuing education, workshops, conferences, and independent learning.
- Maintain up-to-date knowledge of relevant regulations, standards, and best practices for serving the client population.
- Attend interagency meetings and represent the agency in community and provider forums as needed.
- Collaborates with referring agencies, funders, and all related internal staff to effectively support individual client needs.
- Adheres to all applicable federal and state laws and regulations including, but not limited to, those governing confidentiality, privacy, program, billing, and documentation standards.
- Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan.
- Other duties as assigned.
Qualifications
Education : A minimum of associate’s degree in social work, Human Services, International Relations or related field preferred. Equivalent combination of education and experience will be considered.
Credentials : Valid and clean NYS Driver’s license and the ability to conduct home visits. Valid PPD screening available.
Experience : Preferred minimum 1 – 2 years of experience with case management and community outreach in areas such as but not limited to persons affected by substance use disorder, geriatrics, mental health, and refugees. Knowledge of Medicaid, SNAP, Social Security, and legal systems are helpful. May require performance of duties in various community locations and community settings that may not be handicapped accessible.
